For your convenience you may request certain services without coming into the credit union offices. Simply print out the form you need and then fax or e-mail it to us. In most cases we will respond within one business day. Please note special instructions for some forms.

CHECKING AND ATM SERVICES
1. Debit Card Application: Enjoy direct access to your checking funds for purchases, cash and ordering online with a DGEFCU Debit Card.

2. ATM Card Application: If you don’t have a checking account, you can make cash withdrawals from your savings account at an ATM with an ATM card.

3. ATM/Debit Card Transaction Dispute Request: Use this form if you want us to investigate a transaction on your account that you want to dispute. Please note this process may take up to 10 business days.

4. Share Draft (Check) or ACH Stop Payment Request: You may place a stop payment on a check drawn on your checking account or an automatic debit from your account using this form.

5. Credit Union Official Check or Money Order Stop Payment Request: If we have issued you an official check, or you have purchased a money order from us that you would like to place a stop payment on, use this form.

WIRE TRANSFER OF FUNDS
1. Authorization to Wire Transfer Funds: This form will allow you to wire funds from your DGEFCU account to someone else, foreign or domestic. There is a fee for this service.

2. Wiring funds to DGEFCU: Use this form to wire funds into your DGEFCU account. You must provide information regarding your account to the financial institution holding the funds to be transferred into your account.

3. Western Union Wire Request Form: This form will allow you to use Western Union to make a transfer, foreign or domestic. There is a fee for this service.

ACCOUNT MAINTENANCE
1. Payroll Distribution Form: You may have your payroll deduction distributed to savings, checking and/or loan accounts. Use this form to tell us how you wish to have funds distributed from each paycheck/payroll we receive.

2. Change of Member Address or Name: Don’t forget to let us know when you move or have a name change. Use this form to keep your account information with us current.

3. Account Beneficiary Designation: You will save family members inconvenience by designating authority to access your account in the event of your death with this form.

4. Request to Remove a Joint Owner: You may change the joint ownership of your account using this form. Please note that we must retain a signed original in our files.

5. Loan Payoff Release Form: With your authorization on this form, we will release the amount required to pay off a loan you have with the credit union.

6. ID ResCU Member Benefit Selection Form: Use this form to decline the automatic Identity Theft Protection coverage with associated $2.00 monthly maintenance fee.

7. Skip-A-Payment Form: Put a little back this month with a Skip-A-Payment. Fill this form out to skip payment on any of your DGEFCU loans for one month.
